Bee Busy Learning Academy, Inc. (Bee Busy) offers a variety of screening tests for communicable diseases including: the HIV antibody test, the Rapid Plasma Reagin test which screens for Syphilis, the Hepatitis C (HCV) antibody test and the APTIMA GC/CT test that screens for both Gonorrhea and Chlamydia.

The HIV (Human Immunodeficiency Virus) antibody test detects antibodies to HIV AND NOT THE VIRUS ITSELF. Antibodies are created as a natural response to infection. A positive antibody test indicates that a person is infected with HIV and is capable of transmitting the virus to others. A positive test result does not indicate that a person has or ever will have AIDS. Persons who are recently infected with HIV may test negative for HIV antibodies. For this reason, it may be necessary to repeat the test at a later date. Bee Busy offers both the blood test and the Rapid finger prick tests for HIV.

The Rapid Plasma Reagin (RPR) test is a non-treponemal test for the serological detection of syphilis. A positive test indicates that a person currently has syphilis or has had syphilis in the past. A medical professional will determine, by reading the Titer and obtaining a treatment history, if the syphilis-positive person requires treatment. Persons who are recently infected with syphilis may test RPR negative. For this reason, it may be necessary to repeat the test at a later date.

The HCV (Hepatitis C Virus) antibody test detects antibodies to HCV AND NOT THE VIRUS ITSELF. Antibodies are created as a natural response to infection. A positive antibody test indicates that a person is infected with HCV and is capable of transmitting the virus to others. Persons who are recently infected with HCV may test negative for HCV antibodies. For this reason, it may be necessary to repeat the test at a later date.

The APTIMA GC/CT assay test detects Gonorrhea and Chlamydia rRNA in urine samples collected from symptomatic and asymptomatic individuals. A positive test indicates that a person currently has Gonorrhea or Chlamydia and has the potential to pass the bacteria to other partners. Persons who are recently infected (less than two weeks) with Gonorrhea or Chlamydia may still test GC/CT negative. For this reason, it may be necessary to repeat the test at a later date.

Confidential Testing means that your test result(s) will have your name on it. Under the laws of the State of Texas, persons testing positive for HIV and Syphilis will have their name and identifying information (address, birth date, sex, and race/ethnicity) reported to the Houston Department of Health and Human Services and/or the Texas Department of State Health Services (DSHS). This information is not public information and violations of confidentiality are punishable by law. Other than the legal requirement to report persons testing positive for HIV or Syphilis to the local health department, confidential results cannot be released to anyone without your written consent. You must return in person and present your return receipt in order to obtain your results.

Client Grievance Procedure (Appendix A)

1. The client has the right to voice grievance regarding the treatment of care that is (or fails to be) furnished or regarding the lack of respect for property or person by anyone who is providing services on behalf of Bee Busy Learning Academy, Inc.

2. All grievances will be forwarded to the Quality Assurance Specialist (QAS). The client will be required to complete an Incident Report Form or provide their grievance in writing. (All grievances must be documented on an Incident Report Form or other document, even if received orally and signed by the client, if available. Otherwise by the person completing the form). A copy of the Incident Report Form or written grievance will be forwarded to the Compliance Officer (CO).

3. The CO and QAS will only initiate an investigation after the client grievance have been reported in writing.

4. The CO and QAS has (3) three business days after the grievance is filed to perform an investigation. If more time is needed for the investigation, the CO will notify the client in writing stating a more detailed investigation is required.

5. The CO and QAS will conduct the investigation by interviewing all parties involved, compile evidence and draft a conclusion of their findings. The CO will draft a corrective action plan, if necessary.

6. The CO will contact the client with their findings both verbally and in writing. The Executive Director will receive a copy of the letter.

7. The CO maintains a separate confidential file inclusive of the complaint, investigation, and corrective action plan.

8. The CO will make every attempt to resolve the client’s complaint. In the event the client’s grievance has not been resolved, the confidential file will be submitted to the Executive Director for resolution.

9. The Executive Director will contact the client with the final resolution both verbally and in writing. The Board of Directors will receive a copy of the letter. All grievances are forwarded to the Board of Directors. The Board of Directors will follow-up with the Executive Director to ensure the corrective action plan is implemented to client satisfaction.
